URL for database: When exporting references from library databases, you may have to manually put in the database.

Including the "retreived" date: There is a setting that can be turned on that will get RefWorks to remember the retrieved date.

Go to Customize > Import Options > Choose "Yes" for "Show Option for Global Edit Of Electronic Source Related Fields After Import."

When this is selected and a user imports a reference, the option "Edit Imported References" will appear on the "Import References" window. If you click on it, it will give you the option to add the database (sometimes mine adds information there like "RIS Format UTF-8" that I have to paste over) and the retrieved date to the item record. Also, the nice bonus is that you can put records in folders from here.
